You can request a replacement testamur if your original has been lost, destroyed or damaged, or if you have changed your name. You must return your original testamur to Âé¶¹ÒùÔº if it has been damaged, or you require a change of name.
You may request digital or paper replacement documents. The following costs apply:

Digital documents are made available via My eQuals and are only valid when viewed online. If you require a printed copy, please select a paper option.
Digital testamurs are only available for students graduating from 2014 onwards. If you graduated prior to 2014, please select a paper replacement option.
Ensure the email address on this form is your correct personal email address - notification about digital documents will be sent to your personal email address only.

We will be unable to process requests if the appropriate supporting documentation is not provided.
Your request will not be processed unless you upload with this request a scanned copy of a statutory declaration outlining the circumstances of the loss of or damage to the testamur.
